$75–$125 per visit (per diem)
Job Details Overview As a Physical Therapist with Personal Touch Home Care, you will provide direct physical therapy services based on the physician’s plan of care. Your responsibilities include:
Assessing and evaluating patients’ therapeutic, rehabilitative, and functional status.
Participating in the development of comprehensive care plans tailored to each patient’s needs.
Evaluating the home environment to ensure it supports rehabilitation goals.
Assessing muscle strength, mobility, gait, range of motion, and potential for rehabilitation.
Instructing patients and caregivers on the use and care of therapeutic appliances.
Qualifications
Licensed to practice as a Physical Therapist in the state of New York.
Degree from a baccalaureate or master’s program in physical therapy approved by the American Physical Therapy Association, or equivalent credential.
Preferably two or more years of experience as a physical therapist; home‑health experience preferred.
Good verbal and written communication and organizational skills.
Employment Type Per diem, part‑time or full‑time (1099 contractor)
