Base pay range $40.00/hr - $47.50/hr

Responsibilities

Ensure appropriate reimbursement for patient care services.

Participate in the Patient Assessment Process through completion of MDS and Care Area Assessments (CAAs).

Assist in development of individualized patient care plans, identify patient needs and projected outcomes per federal and state regulations.

Contribute to the nursing center interdisciplinary Resident Assessment Instrument (RAI) process, following RAI guidelines for completion, computer encoding and electronic transmission of the MDS.

Identify changes in patient condition with interdisciplinary team and coordinate scheduling and completion of indicated assessments.

Assist with Activities of Daily Living training for new hires and ongoing continuing education for existing staff.

Attend educational programs to maintain knowledge of current RAI/Prospective Payment System regulations.

Qualifications

Registered Nurse currently licensed by the state and in good standing.

Previous experience in a hospital, long‑term facility, or other healthcare related facility.

Prior experience completing MDSs and Care Plans.

Knowledge of general, rehabilitative and restorative nursing and medical practices and procedures, and laws, regulations and guidelines governing long-term care.
