Full-time Community Health Nurse Practitioner Needed at Eastside Internal Medicine LLC in New Rochelle, NY You will have the opportunity to provide holistic care to individuals and families in the community, addressing their healthcare needs and advocating for equitable access to quality services. Collaborating with community organizations and implementing health education initiatives, you will play a key role in empowering individuals and improving community health outcomes. Work Schedule: Hours from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach events or health fairs during weekends may be required. Approximately 40 hours per week. Responsibilities: Conduct health assessments and screenings for individuals and families. Diagnose and manage acute and chronic conditions, including preventive care. Develop and implement health education programs with community organizations. Provide culturally sensitive care and advocate for patients’ rights. Coordinate referrals and follow-up care for continuity. Maintain accurate medical records in compliance with standards. Participate in outreach events and health fairs to raise awareness. Stay updated with community health trends and evidence-based practices. Requirements: Current Nurse Practitioner certification. Active state nursing license. Master’s degree in nursing from an accredited program. 3-5 years of clinical experience, preferably in community or public health. Strong interpersonal and communication skills. Commitment to social justice, health equity, and advocacy for vulnerable communities. Salary and Benefits: Salary range of $120,000-$220,000 annually, based on experience.
