This is a full‑time, full‑time position based in the Santa Clara County community covering locations such as Campbell, Cupertino, Gilroy, Los Altos, Los Altos Hills, Los Gatos, Milpitas, Monte Sereno, Morgan Hill, Mountain View, Palo Alto, San Jose, Santa Clara, Saratoga, and Sunnyvale. Compensation We compensate on a pay‑per‑visit basis. Rates range from $70/visit to $140/visit, commensurate with experience and visit type.
Responsibilities
Provide treatment as ordered by the attending physician, dentist, or podiatrist.
Assist in developing and updating the plan of treatment or Plan of Care (POC) in an electronic medical record system.
Supervise 1 to 2 Physical Therapy Assistants, ensuring productivity standards are met.
Evaluate patients' level of function and report findings to the attending physician and case manager.
Advise, consult, and instruct family members and Lorian personnel on patients' therapy program.
Participate in in‑service education programs to enhance professional development.
Qualifications
Graduate of a physical therapy school approved by the Council of Medical Education of the AMA or its equivalent.
Licensed or registered to practice as a PT in the state of practice.
Preferably at least two (2) years of experience as a PT supervising a Physical Therapist Assistant in Home Health.
Extensive and diverse training in areas such as orthopedic, neurology, cardio‑pulmonary, etc.
Ability to read and follow written instructions and document care given.
Self‑directing and able to work with little direct supervision.
Possesses and maintains current CPR certification.
Flexible and cooperative in fulfilling role obligations.
Demonstrates tact, patience, and good personal hygiene.
Licensed driver with an insured automobile in accordance with Lorian Health requirements.
Ability to verbally communicate in the language of the client and document all client‑related activity practices.
