About the Position The Admission Nurse is responsible for the care of patients assigned by the Director of Hospice Admissions. The RN will perform admission assessments, identify patient needs, and under the physician’s nursing order devise a Plan of Care to address all patient needs.

Job Duties

Presents hospice services and philosophy of care to patients and families facing end‑of‑life care decisions.

Performs in‑person assessments to determine eligibility for hospice, obtains certification from the hospice physician and consent for service.

Completes the admissions process, initiates a care plan, and ensures immediate care needs are addressed.

Communicates with the receiving interdisciplinary team about goals of care, initial plan, and patient/family expectations.

Provides direct nursing care in a hospice setting under the physician’s order.

Performs and completes admission/start of care (SOC), recertification, evaluations, discharges, and documentation in a timely manner.

Performs a detailed assessment resulting in a patient‑specific Plan of Care; documentation follows agency policy.

Communicates with team members regarding the Plan of Care and continues collaboration for the duration of the case.

Effectively communicates with referring physicians, providing accurate treatment notes.

Documents interpretation of hospice care policy, agency philosophy, objectives, and long‑range plans.

Prepares for patient discharge planning as early as possible and informs all parties.

Records and reports changes in patient condition promptly to physicians and team members.

Recommends additional services as needed.

Advise and consult with families on the patient’s therapy program.

Attend staff meetings at least four times per year.

Document all communication with other personnel to support case conference notes.

Use equipment and supplies efficiently and request necessary items.

Maintain confidentiality of patient information and co‑worker personal data.

Implement infection control and safety measures as appropriate.

Address patient concerns consistently; promote education for patients, family members, and caregivers.

Demonstrate current professional knowledge.

Attend mandatory in‑services and maintain 100 % compliance with data‑based training.

Participate in orientation of hospice/home health staff members.

Immediately report abuse or suspected abuse to supervisor and relevant authorities.

Job Requirements

Graduate of an accredited nursing school.

Licensed to work as a Registered Nurse in Virginia.

At least one year of acute care experience; hospice preferred.

Current CPR certification.

Effective English communication skills.

Knowledge of home health and hospice care preferred.

Willingness to learn and perform HIS documentation and SOC.

Ability to make independent decisions and work independently throughout the workday.

Driver’s license, automobile insurance, and reliable transportation.

Compassionate and tactful interaction with patients, families, and caregivers.

Teamwork orientation—collaborate with all disciplines involved in the patient’s care.
