Overview DaVita is seeking a self-motivated, organized Peritoneal Dialysis Registered Nurse (PD RN) to educate patients and care partners in a clinical setting so they can dialyze at home. You will be responsible for the complete ongoing care of each patient to ensure their treatment stays on track and for independently managing your cases with confidence and accountability. Responsibilities Provide comprehensive patient care in an independent setting and apply core nursing skills to assess, diagnose and implement complete care plans for the whole patient. As a case manager, track and organize your patients and observe the impact of your care. Empower patients by teaching them to administer their own dialysis and engage them in managing their health. Partner with home RN staff to support the growth of the home program, including educating new in-center hemodialysis patients in clinics. Lead, coach and guide with nursing skills; work independently while building deep relationships with patients, support systems and care partners. Schedule Typically, an 8-hour day, Monday through Friday with weekends and holidays off. Flexibility to manage your own cases and schedule; scheduled home visits and on-call shifts vary by program and are compensated accordingly. Requirements Current Registered Nurse (RN) license in the state of practice Minimum of 2 years of dialysis RN experience; PD experience preferred Current driver's license, reliable transportation, and vehicle insurance in state of residence Associates Degree in Nursing (A.D.N.) from an accredited school of nursing required; Bachelor of Science in Nursing (B.S.N.) or a three-year diploma may be substituted for a nursing degree Basic computer skills and proficiency in MS Word and Outlook You might also have (nice-to-haves) Experience in Chronic Hemodialysis, Acute Dialysis, Home Hemodialysis (HHD) or Peritoneal Dialysis (PD) nursing Training/Teaching/Healthcare Education background Previous case management experience Familiarity with or willingness to learn telehealth technologies CNN, CDN, CPDN certification Ready to make a difference?
