Providence Medical Group - Southern Oregon is seeking a board-certified/board-eligible Family Medicine physician to join the Eagle Point Family Medicine Clinic, located just outside of Medford, Oregon. The clinic features a patient-centered medical home model and multi-disciplinary team including physicians, advanced practice providers, psychologist, case manager, and clinical pharmacist.

Position Details
Full-time position, 1.0 FTE (flexible to hire 0.9 FTE to 1.0 FTE)
Schedule can be either 4, 4.5, or 5 days per week in clinic
Outpatient care only with minimal phone call
Compensation & Benefits
$50,000 signing bonus and up to $80,000 in educational loan repayment funds
Guaranteed base salary, incentives, and relocation assistance available
Standard Employed Provider Benefits Package, including PTO, Health, Life, Dental, Vision, Disability, CME days and dollars, and Malpractice
The clinic has an active site application on file with the Oregon Office of Rural Health for those interested in applying to the provider incentive programs
