The Perinatal Nurse Navigator for MedStar Health’s Safe Babies Safe Moms program plays a vital role in advancing our commitment to exceptional, compassionate, and equitable maternal-infant care. Serving as a dedicated clinical resource and advocate, this role supports expectant and new mothers by coordinating care across MedStar Health’s multidisciplinary teams, including obstetrics, pediatrics, behavioral health, social work, and community partners, to ensure seamless navigation throughout the prenatal, delivery, and postpartum experience. The Nurse Navigator proactively identifies patient needs, provides education, facilitates timely follow-up, and removes barriers to care, ultimately improving outcomes, enhancing the patient experience, and strengthening the program’s mission to support healthy beginnings for every mother and baby we serve.
Primary Duties and Responsibilities
Coordinate and guide expectant and new mothers through prenatal, delivery, and postpartum care to ensure seamless transitions and timely follow-up.
Serve as a trusted clinical resource and patient advocate, offering education, clarification of care plans, and support throughout the perinatal journey.
Conduct proactive outreach and assessments to identify medical, social, and behavioral health needs, as well as barriers to accessing care.
Facilitate referrals and connect patients to MedStar Health services, such as OB/GYN, pediatrics, behavioral health, and social work, as well as relevant community resources.
Monitor patient progress, track adherence to recommended visits, and follow up on missed appointments to support continuity of care.
Collaborate closely with multidisciplinary teams to align care plans, participate in case discussions, and ensure a coordinated approach to maternal and infant health.
Support high-risk patients by coordinating rapid follow-up, additional resources, and increased touchpoints to promote safety and positive outcomes.
Maintain accurate, timely documentation and contribute to program reporting, data tracking, and quality improvement initiatives for Safe Babies Safe Moms
Education
Bachelor’s degree in nursing required
Master’s degree in nursing preferred
Experience
Minimum of three (3) years of recent RN experience in obstetrics, labor and delivery, maternal–child health, or women’s health is required.
Experience providing patient education, care coordination, or case management within a healthcare setting.
Familiarity with high-risk perinatal populations and the ability to address medical, social, and behavioral health needs.
Demonstrated experience working within multidisciplinary teams and collaborating with physicians, nurses, social workers, and community partners.
Experience using electronic health records (EHR) for documentation and care tracking.
Licensure & Certifications
Active, unencumbered Registered Nurse (RN) license in the District of Columbia is required.
Basic Life Support (BLS) certification from the American Heart Association (AHA) required.
Advanced Cardiac Life Support (ACLS) preferred
Electronic Fetal Monitoring (EFM) Certification preferred but not required.
Maternal Newborn Nursing (RNC-MNN) or Inpatient Obstetric Nursing (RNC-OB) certification is a plus.
Care Management or Case Management certification (e.g., CCM, ACM) is preferred but not required.
