Qualifications Minimum Requirements
Licensed to practice medicine in accordance with Chapter 458, Florida Statutes*
Four years of appropriate experience. Completion of a residency program may substitute for the required experience.
Preferred Qualifications
Certification/Licensure in the state of Florida
Federal DEA Certification
Medical Degree – M.D., D.O., or foreign equivalent
Board certified in Family Medicine
Completion of an accredited residency program
