Seeking a board certified Palliative Care Physician for Medical Director role in Hartford, CT.
Responsibilities
Lead a team of hospice physicians and per diem APRNs caring for patients across Connecticut
Participate in the decision-making process in relation to admission to the hospice (homecare) or inpatient setting
Participating as a member of the Interdisciplinary Team in the development of individualized care plans
Certify (re-certify when necessary) patients eligible for hospice care under the Medicare Hospice Benefit
Consult with attending physicians regarding pain and symptom management
Coordinate efforts with the attending physician to provide continuity of care
Develop and monitor the medical component of the hospice program
Participate in the development, coordination and implementation of hospice services and policies
Participating in the development of staff and volunteer training
Conducts Face to Face visits for hospice admissions and recertification when appropriate as required by CMS.
Act as a hospice liaison and advocate with the national, state and local physicians, healthcare professionals, partners and community
Perform related administrative functions as required and/or assigned
Participating on the Professional Advisory Committee
Establish health policies for employees when requested
Participate in the legislative process as it relates to hospice
Attend / participate in Hospice QAPI Team meetings and initiatives as requested
