Position Summary Evaluates medical records, provides clinical and surgical abstraction and assigns appropriate clinical diagnosis and procedure codes in accordance with nationally recognized coding guidelines.
Core Functions
Analyzes medical information from records, accurately codes diagnostic and procedural information according to national coding guidelines, and addresses NCCI edits. Provides timely and accurate coding to meet departmental productivity and quality standards.
Abstracts clinical diagnoses, procedure codes, and other pertinent information into the electronic medical records. Seeks missing information and creates complete records, including necessary signatures and authorizations.
Provides quality assurance for medical records, ensuring compliance with coding rules and regulations (state Medicaid, CMS, OIG, HCFA, and industry standards).
Compiles daily and monthly reports and tabulates data for research or analysis purposes.
Works independently under regular supervision, using specialized knowledge for accurate coding of ICD/CPT codes and seeking guidance on coding guidelines and LCDs.
Minimum Qualifications
High‑school diploma/GED or equivalent; or an associate’s degree in a related health‑care field.
Certification: CPC, CCS, CCS‑P, CCA, CPC‑A, RHIA, or RHIT in an active status with AHIMA or AAPC.
Six months of professional coding services or related healthcare experience in diverse facilities.
Knowledge of ICD and CPT coding principles, demonstrated by certification.
Ability to work effectively in a remote setting using common office programs and coding software.
Preferred Qualifications
Specialty certification in urology, surgical urology, gynecology oncology, or academic coding guidelines.
One year of recent E/M urology coding experience (clearly reflected in resume).
Surgical urology experience and better than CPC‑A level certification.
