Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC)

to join our team at a long-term care facility in the

Greater Chicago area . This is an

in-person position

providing psychotherapy and mental health support to residents in a long-term care setting. The ideal candidate will have strong clinical skills, a collaborative mindset, and a passion for improving the emotional and psychological well-being of older adults and individuals with complex health needs. Responsibilities Conduct comprehensive mental health assessments and develop individualized treatment plans for residents. Provide ongoing psychotherapy and counseling using evidence-based approaches such as CBT, motivational interviewing, and person-centered therapy. Facilitate individual and group therapy sessions to address depression, anxiety, grief, adjustment issues, and other behavioral health concerns. Collaborate with facility staff, nursing teams, and physicians to ensure integrated and holistic care. Maintain accurate, timely, and compliant documentation, including assessments, progress notes, and treatment updates. Participate in care planning meetings and provide behavioral health insights to support residents’ overall well-being. Support residents and families with education, coping strategies, and guidance on available community resources. Requirements Master’s degree in Social Work (MSW) from an accredited institution. Current

Illinois LCSW or LCPC

license in good standing. Experience providing psychotherapy, preferably in long-term care, hospital, or medical settings. Strong understanding of mental health diagnoses, treatment planning, and documentation standards. Excellent interpersonal and communication skills, with the ability to work collaboratively in a multidisciplinary environment. Familiarity with EMR systems and HIPAA compliance. IM+CANS certification is strongly recommended
