155 Addison Street Position Summary:
Nursing The Home Care Primary Nurse is responsible for overseeing, assessing, planning, and coordinating nursing care delivered to participants. The nurse may delegate some tasks but retains responsibility for ensuring care aligns with the care plan and medical provider orders. Responsibilities include providing and documenting quality patient care in collaboration with the Interdisciplinary Team, with care provided in the PACE Day Health Center or in participants' homes. Essential Duties and Responsibilities: Assess patients through history, tests, observations, and consultation with the team. Prepare and present patients for rapid provider review and treatment. Implement care plan interventions, testing, treatments, or provider orders. Oversee delegated tasks, monitor outcomes, and adjust care plans as needed. Administer medications, tests, and treatments safely and effectively, managing medication refills with provider authorization. Educate patients and families about health care regimens, disease management, medications, and wellness. Qualifications and Requirements: Baccalaureate degree in nursing preferred. Current Massachusetts nursing license. Current BLS certification. Minimum 2 years of experience in ambulatory, home health, hospital, or sub-acute settings preferred; equivalent experience considered. At least one year experience working with a frail or elderly population. Skills/Abilities: Basic assessment and triage skills. Effective communication of assessment findings verbally and in writing. Ability to communicate effectively with staff, providers, participants, and families; bilingual skills a plus. Excellent customer service skills. Flexibility, initiative, creativity, and organizational skills. Basic computer skills, including word processing and email; proficiency with Windows-based programs and EMR systems. Pay Range:
Starting at $40.00 up to $60.00 depending on experience.
