Job Posting: MEDICAL TRANSPORTATION COORDINATOR Location: 1099 JAY ST Employment Type: Part-Time Schedule: 10 hours a week, working Monday (5 hours) and Thursdays (5 hours) Salary: $19.01/hr General Description Coordinate transportation services for people living with HIV/AIDS to medical appointments and medical services within Monroe and surrounding counties.

Essential Duties and Responsibilities

Complete initial intake and assessment paperwork for eligible participants and assess their transportation needs to ensure they are grant qualified.

Complete reassessments and essential documents with client within required time frame.

Arrange for appropriate level of transportation assistance for eligible clients.

Provide transportation to clients in a welcoming and friendly manner to and from grant approved medical and non-medical appointments.

Input data into AIRS system and other program databases.

Document services in appropriate electronic health record and submit copies of electronic data base entries to supervisor bi-weekly to ensure accuracy.

Monitor and coordinate the maintenance of the Regional medical transportation van and report to supervisor accordingly. Ensure MT van tracking form and receipts are submitted to supervisor by the 6th of the month.

Purchase and distribute bus pass/gas cards. Document the distribution of bus pass/gas cards efficiently when distributing to client (face to face) or via mail and on MT distribution log.

Establish and maintain ongoing positive collaborative relationships with medical and transportation providers.

Process invoices as generated by transportation providers and document verified trips on the travel taxi expense log.

Assist in completing mandatory and required reports for program by funder and supervisor respectively.

Refer client to case management services as appropriate and in a timely manner.

Case conference and coordinate services with existing case management services and medical providers and subcontractors.

Monitor status of funds available for all transportation assistance lines.

Assist in identifying additional resources for client transportation needs.

Participate in team meetings, staff meetings, AI Medical Transportation Provider Quarterly Webinar, and case conferences.

File case logs and maintain clients’ files/records in a timely manner per supervisor’s direction.

Attend all mandatory training.

Ensure compliance with all applicable local, State and Federal regulations and agency policies.

Represent Catholic Charities Family and Community Services on appropriate boards and committees as assigned by supervisor.

Participates in relevant Agency meetings and/or trainings

Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.

Qualifications Education:

Associate degree in health or human services, business, or computer related field preferred. High School diploma or GED required.

Experience:

At least two years of human services experience or a combination of customer service with office or computer related experience required
