FlexStaff Clinical Manager - Integrated Care

FlexStaff is seeking a qualified Clinical Manager - Integrated Care for two locations: Brooklyn and Bronx. This role ensures effective completion of clinical assessments, including onboarding, reassessment, UAS-NY, comprehensive assessments, medication reviews, and post-hospital discharge evaluations. The Clinical Assessment Manager will adhere to Department of Health policies regarding UAS-NY and related workflows, serving as a resource for documentation and evaluation of clinical information in the electronic record system. Job Responsibilities

Coordinate with intake, enrollment, and care teams for assessment appointments, utilizing UAS-NY, and explain program benefits to clients. Conduct in-home assessments to determine eligibility for CBLTC services, ensuring documentation accuracy. Review and update assessments, diagnoses, and medication information in EMR, escalating significant changes. Promote the program to referral sources and educate clients on benefits and services. Escalate assessment questions or concerns to the quality review team. Complete and reassess PCAT Tasking Tool, providing recommendations and escalating issues. Develop service plans during home visits and utilize Lenavi analyzer as needed. Document interactions and outcomes promptly in the designated systems. Maintain knowledge of CMS/PACE guidelines and organizational policies. Remain accessible via phone/email for client staff inquiries. Work some weekends and travel to various boroughs. Provide clinical information to facilitate enrollment and coordinate with site leadership and IDT. Serve as a clinical point person post-enrollment and participate in quality improvement activities. Review performance metrics to enhance processes and ensure a minimum of 2-3 assessments daily. Operate within the scope of practice and meet competency standards. Participate as a member of the IDT and perform other duties as assigned. Schedule

8:30AM - 5:30PM, 40 hours/week. Qualifications

BS in Nursing or related health field preferred. Minimum 1 year working with frail or elderly populations; additional training provided if less than 1 year but other requirements are met. 3-5 years in managed care programs such as MLTC, LTHHCP, MAP, PACE. Experience working independently in a clinical setting. Over 5 years in care management, disease management, or related fields. Proficiency with UAS-NY, Lenavi, CRM platforms, and clinical systems. Current NYS Registered Nurse license required. Legally authorized to practice in NY, with up-to-date immunizations and clearance. Strong communication, interpersonal, and clinical management skills. Proficiency in Microsoft Office and report writing. Ability to multitask, adapt, and work flexibly, including some weekends.
