Key Responsibilities Conduct comprehensive psychiatric evaluations and mental status assessments for residents in skilled nursing, assisted living, and rehabilitation settings. Design, implement, and monitor individualized treatment plans, including psychotropic medication regimens and behavioral interventions. Reassess and adjust care strategies based on clinical progress and patient feedback. Collaborate closely with physicians, psychologists, nurses, and social workers to deliver integrated, holistic care. Maintain accurate, timely, and compliant documentation within the electronic health record (EHR). Contribute to case conferences, facility rounds, and quality improvement initiatives to strengthen care outcomes. Qualifications
Active New York State Nurse Practitioner license and PMHNP certification. Prior experience in adult or geriatric psychiatry preferred (new graduates welcome). Proficiency in psychopharmacology and evidence-based treatment modalities. Strong independent clinical judgment and ability to manage a consistent caseload. Excellent communication and collaboration skills across multidisciplinary teams. Comfortable using EHR systems and digital documentation tools.
