Practice Setting: Outpatient Primary Care – Community‑Focused

Key Responsibilities
Deliver full‑spectrum outpatient family medicine care to patients of all ages
Perform routine wellness visits, chronic disease management, preventive care, and acute care services
Maintain accurate and up‑to‑date documentation in the electronic health record (EHR)
Collaborate with an interdisciplinary team including advanced practice providers, nurses, care coordinators, and behavioral health professionals
Participate in quality improvement efforts and care initiatives focused on community health
Foster an inclusive, respectful, and supportive environment for patients from diverse backgrounds
Qualifications
MD or DO with board certification (or eligibility) in Family Medicine
Active Georgia medical license or ability to obtain
Current DEA license
Passion for working with underserved communities
Strong communication skills and a commitment to team‑based care
