Physical Therapist Case Manager: Home Health
Base pay range: $120,000.00 - $135,000.00 per year. Additional annual bonus up to $10,000.

Role Overview Part‑time Physical Therapist Case Manager based in San Mateo County. In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, ensuring they can live safely and productively.
Key Responsibilities
Perform home evaluations and develop individualized care plans.
Help patients with mobility, movement, and regain independence.
Create detailed treatment and care plans for each patient.
Communicate and coordinate with patients’ physicians to ensure high‑quality care.
Evaluate and report patients’ progress to MD.
Supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards.

Qualifications
Current unrestricted CA Physical Therapist license.
Current BLS/CPR certification.
Valid CA driver’s license.
Knowledge of local community resources.
Knowledge of health‑care culture and patient behavior.
Knowledge of state and federal home‑care regulatory guidelines (preferred).
Experience in home health care (highly preferred).
Experience with high‑acuity patients (plus).
Experience working with interdisciplinary team (highly desired).
Case Management fundamentals (highly desired).
Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership, organized.

Additional Information Wage range for Physical Therapist, Level 1: $120,000.00 – $135,000.00 plus a $10,000 annual bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and a 401(k) (up to 4% match).
As a practice, most PT Case Managers are hired as PTIs. Once an PTI can supervise two PTAs, take on a larger case load and assume higher decision‑making autonomy, the individual is promoted to a PTII with a minimum salary of $135,000 plus a $20,000 bonus (paid quarterly).
