What you will do:
Provide embedded specialty care in Federally Qualified Healthcare Centers (FQHC) to increase access for community members in places they already know and trust.
Improve health outcomes
Reduce health disparities by eliminating barriers to healthcare that disproportionately impact under-resourced communities.
Work within Community CARE, a circle of programs that exists to honor the human dignity of all by building connections that expand the reach and definition of healthcare.
Provide holistic care by coupling medical care with dedicated support services in the areas of nutrition, clinical case management, and social determinants of health.
What you will need:
IL medical license or pending
DEA/Controlled Substance License or pending
Education:
Gastroenterology Fellowship
Certification:
Board Certified or Board Eligible
