Registered Nurse (RN) – Home Health (Day Shift)
Location: Washington, DC (Potomac, DC 20640)
Pay: $57/hr (40 hours guaranteed)
Base Pay Range: $55.00/hr - $57.00/hr
Position Overview: We are seeking an experienced Home Health Registered Nurse (RN) to provide comprehensive care to patients in their homes. The RN will deliver high-quality, patient‑centered care, educate patients and families on self‑management, and ensure accurate documentation in accordance with Medicare‑certified home health standards.
Responsibilities
Provide in‑home nursing care to adult and geriatric patients with chronic and acute conditions.
Manage an average of 8 patient visits per day.
Perform assessments, admissions (SOC/ROC), recertifications, revisits, and discharges.
Administer IV infusions, manage central lines, ports, and peripheral IVs.
Provide wound care, tracheostomy care, ventilator maintenance, and medication management.
Complete OASIS E documentation in Homecare HomeBase (HCHB).
Collaborate with interdisciplinary teams including PT, social work, and phlebotomy.
Maintain compliance with infection control and facility standards.
Requirements
Minimum 2 years of recent Home Health experience.
Medicare‑certified home health agency experience required (must be verifiable at medicare.gov).
OASIS E and Homecare Home Base (HCHB) experience required.
Certifications & Licensure
Active DC RN License (must be in hand upon submission).
BLS (AHA) – required.
COVID‑19 vaccine – not required.
Offered pay rate will be based on education, experience, and healthcare credentials.
