Job Summary-
Community Missions has various Lead Residential Counselor positions throughout the agency; in both youth and adult services. The positions are responsible for the implementation of trauma informed and person-centered individualized service plans. In addition, they supervise daily activities and maintain a safe and supportive atmosphere for the residents. Lead Residential Counselors work closely with residents, their families, and a network of service providers to ensure the residents receive the necessary support to achieve their personal goals.
Key Responsibilities-
Conduct assessments of residents’ strengths and needs. Develop and implement individualized care plans that address resident needs and goals Monitor resident progress through regular follow-ups and adjust care plans as needed Advocate for residents to ensure they receive appropriate services and support. Maintain accurate and up-to-date case files and documentation in compliance with regulatory requirements and policies. Assist residents with daily living activities, such as meal preparation, medication management, and cleaning. Provide crisis intervention and support to residents in emergency situations Ensure compliance with all relevant policies, procedures, and regulations On-call responsibilities in rotation with a team (for some positions) Hours include evening, weekend, and holidays.
Qualifications-
Bachelors degree in psychology, social work, counseling, or a related field plus 1 year professional experience in the human services field Valid New York State Driver’s License with clean driving record
