Nurse Care Manager, Geriatrics, 40 Hours (Days)
Position Summary:
Performs a variety of high‑level nursing case management services through the development and use of geriatric nursing practice standards. Activities include conducting a comprehensive geriatric nursing assessment, history and physical; developing and reassessing a plan of care which addresses the physical and psycho‑social needs of the older adult and family; patient/family education; and coordination of care and services across various sites of care. The Geriatric Nurse Case Manager assists in the development, implementation, evaluation and revision of the case management model of care and program, ensuring desired patient outcomes are achieved and that variances from established best practices are evaluated and addressed as necessary.
Essential Responsibilities / Duties Patient Care:
Delivers direct patient care via home visits and/or hospital visits, as required.
Utilizes telephone triage to evaluate patient care issues, identifying actual and/or potential illness or change in condition that may impact the care plan. Makes recommendations to address identified problems.
Provides proven methodologies and recommendations for successful implementation of the care plan, including care coordination, activities of daily living, legal, financial management, health‑care delivery and reimbursement systems, prescription drug cost management, and long‑term care insurance.
Advocacy to ensure elder safety, physical, psycho‑emotional, legal and financial well‑being, supporting care‑giving and quality of life goals; navigating the healthcare system and obtaining third‑party reimbursement for covered goods and services.
Assessment and monitoring of care delivered to the elder, providing recommendations to modify the care plan or providers/vendors as necessary.
Consultation and conferencing with the elder, their caregivers, family, and representatives to assess issues, consolidate information, and coordinate planning and care delivery.
Ensures safe and appropriate transitions of care using effective written and verbal communication skills and expert knowledge of community and long‑term care resources.
Participates in the development, implementation, evaluation, and revision of the nursing case management program in Geriatrics.
Patient Education & Support:
Communicates with patients, their representatives, and providers to effectively implement plans and provide expertise regarding ongoing eldercare issues and services.
Facilitates problem solving and decision‑making by providing guidance and support to patients, families, acute care providers, and community health care providers regarding prognosis and course of care in acute and community settings.
Program Management & Administration:
Collaborates with administration, physicians, and other colleagues in Geriatrics and Medicine to develop, revise, and update critical paths and support development of DRG‑specific standards.
Evaluates financial variance and quality improvement data to determine focus areas for the nursing case management model and program; assists in developing and monitoring financial goals for targeted case‑managed patients and optimizing reimbursement of services.
Develops professional relationships with the community through effective networking and professional service, fostering a positive public image for the Geriatric Service and Boston Medical Center.
Contributes to the education of nursing students and other health‑care disciplines with a focus on geriatric nursing and the case manager role.
Expands geriatric practice base through nursing research to maximize high‑quality, cost‑effective patient outcomes; performs ongoing evaluation of program outcomes and identifies opportunities for improvement.
Professional Development:
Identifies own learning needs and maintains current knowledge, competency, and expertise in the specialty.
Completes annual mandatory educational requirements, departmental/specialty competencies, and maintains appropriate credential status.
Participates as a committee member and/or chair when requested by geriatric management team.
General:
Conforms to hospital standards of performance and conduct, including patient rights, to provide the best possible service and care.
Utilizes hospital’s behavioral standards for decision‑making and supporting department and hospital goals.
Follows established infection control and safety procedures.
Performs other duties as needed.
Education & Qualifications Education:
Graduate of an accredited baccalaureate nursing program is required; masters preparation highly desirable.
Certificates, Licenses, Registrations Required:
License to practice professional nursing as a registered nurse in Massachusetts.
Certificate in Geriatric Nursing highly desirable.
Experience:
5 years experience as an RN in a care‑delivery model that includes case management; experience with geriatric populations preferred.
Ability to make home visits for nursing assessments; home‑care experience is a plus.
Knowledge And Skills:
Self‑directed, self‑starter.
Analytic ability to solve clinical nursing issues.
Organizational skills to prioritize and efficiently complete workload.
Physical ability to meet core job responsibilities in the practice setting.
Ability to manage stress due to critical patient care issues, changing organizational climate, and staffing issues.
Effective verbal and written communication skills appropriate to patient populations.
Ability to apply principles of adult learning to educate staff, patients, and families.
Multilingual skills beyond English (preferred).
Proficient with Microsoft Office (Word, Excel, PowerPoint, Outlook) and web browsers; experience with EPIC electronic medical records.
Compensation $104,000.00 – $151,000.00 This range reflects base pay for the minimum job qualifications and is determined by education, experience, skills, and certifications. Boston Medical Center also offers comprehensive benefits, including medical, dental, vision, pharmacy, discretionary bonuses, merit increases, 403(b) savings match, paid time off, and career advancement opportunities.
