Role Description This is a full-time on-site role for a Hospice Liaison located in Montrose, CA. The Hospice Liaison will be responsible for building and maintaining relationships with healthcare professionals, patients, and families to promote hospice care services. Responsibilities include educating the community about hospice and end-of-life care options, coordinating patient referrals, and developing strategies to expand hospice and palliative care outreach. The role also involves collaborating with clinical staff to ensure seamless transitions of care for patients.
Qualifications Understanding and experience in Hospice Care, End-of-Life Care, and Palliative Care Strong Sales skills and experience working in a healthcare or hospice-related environment Knowledge and experience in Home Care services Excellent interpersonal, communication, and organizational skills Ability to collaborate with healthcare professionals, patients, and families Bachelor’s degree in a related field is preferred Experience in healthcare sales or marketing is a plus Valid driver’s license and reliable transportation for local travel
