Post Acute Care Opening in Utica, NY Syracuse 55mWe are seeking a full-time primary care physician. Your role will include patient care rounding in a post-acute care setting in a skilled nursing facility located in the Utica, NY, area. You will round 5 days a week with the flexibility to choose your rounding schedule. Day call only on days covering Day call weekend rotationOverview Primary care rounding in skilled nursing facilities Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care Collaborate with clinicians, facility leaders and others regarding care and services Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery Provide oversight, guidance, and mentorship to our nurse practitioners Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care Collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issuesQualifications Minimum of 1 year primary care experience Post-acute care experience a plus, but not require Hospitalist, internal medicine, family medicine, emergency medicine and related specialties encouraged to apply. Active and unrestricted New York license or application in process Active and unrestricted DEA/prescriptive authority or application in processCompensation Expected compensation is $183,222 to $221,114 annually with no cap on productivity income potential.
