Role Summary The FAST (FA Specialist Team) Registered Nurse is part of Entyre’s dedicated intake team responsible for executing flawless First Assessments (FAs), eliminating erroneous disqualifications, accelerating time to placement, and ensuring families receive decisive, expert guidance during the intake process. This team operates at high velocity and delivers “insanely great” first‑touch experiences for families by ensuring every FA is completed early, accurately, and with expert navigation of regulations. FAST Registered Nurses work in tightly coordinated RN–CM pairs but operate on a centrally managed schedule optimized for efficiency so that we can see as many families as possible. The FAST program exists to drive erroneous disqualifications to zero, ensure assessments are done early and correctly, and accelerate families’ path to approval and placement. This is a high‑performance clinical role designed for nurses who thrive in fast, precise, field‑based work.

What You Will Do

Become experts in what in AFC regulations as well as what it takes for families to qualify for AFC under MassHealth.

Conduct First Assessments (FAs) rapidly and accurately to qualify families for the appropriate level of AFC support.

Partner with families to overcome initial objections (e.g., moving in together, documentation gaps), ensuring readiness for successful enrollment.

Drive erroneous disqualifications to zero through expert judgment, high‑quality documentation, and flawless execution.

Produce clean, audit‑ready clinical documentation for all FAs.

Travel to PCP offices when necessary to collect overdue records or signatures.

Obtain CAC certification and guide families through Medicaid/AFC plan qualification and enrollment.

Collaborate closely with the FAST RN counterpart and FAST Supervisors to ensure assessment quality, compliance, and consistency.

Coordinate with Customer Success scheduling teams to optimize availability for families.

Maintain detailed, accurate, and audit‑ready records for all FA encounters.

How Success is Measured

# of erroneous disqualifications (reviewed by FAST Supervisor)

Average speed to placement eligibility (days from FA to approval & handoff)

Quality of documentation and regulatory compliance

Professionalism and clarity when working with families at sensitive moments

What You Bring

Current RN license in Massachusetts

2+ years of experience in home care, LTC, community nursing, behavioral health, or related settings

Experience with intake assessments, intake, or high‑touch client interactions in healthcare, behavioral health, or community services

Strong clinical assessment skills and comfort with independent decision‑making

High empathy and communication skills for working with vulnerable adults and families

High attention to detail and documentation quality

Valid driver’s license + travel throughout Massachusetts and willingness to be on the road (typically within 60‑90 minute radius of your home city)

Proficiency in electronic health documentation and Microsoft Office/Teams

Comfort operating in a fast‑paced, high‑volume, rigorously measured environment
