for the
DFTA Caregiver Services Program
in Brooklyn, NY. This is an exciting opportunity to lead a community-based program that supports
caregivers and older adults
across New York City.
The Program Director will oversee daily program operations, provide
staff supervision , ensure
contract compliance , coordinate
client care , and expand
community outreach and partnerships . This role is ideal for an
LMSW
or
LCSW
with proven experience in
program management ,
case management , and
aging services . The ideal candidate must have direct experience working with
NYC Department for the Aging (DFTA)
programs – only applicants with prior DFTA experience will be considered.
Key Responsibilities
Lead and mentor a multidisciplinary team of Social Workers, Case Assistants, and interns through individual and group supervision.
Coordinate client intakes, assessments, eligibility screenings, and referrals; ensure timely case follow‑up and delivery of caregiver support services.
Monitor program deliverables, outcomes, and documentation to meet all NYC DFTA and funding requirements.
Conduct presentations, represent the program at public events, and build relationships with community organizations to enhance caregiver awareness and engagement.
Work closely with leadership to identify service gaps, support strategic initiatives, and ensure alignment with agency mission and goals.
Qualifications
Education:
Master’s degree in Social Work (LMSW or LCSW required).
Experience:
Minimum 3 years of experience in clinical social work, case management, or human services.
Prior program management or supervisory experience strongly preferred.
Experience with NYC DFTA programs or services for older adults and caregivers required.
Technical Skills:
Proficient in Microsoft Office Suite and case management or client tracking systems; strong data management and documentation skills.
