- Provider must be Ventricular Assist Device (VAD) Certified (this is for Advanced Heart Failure, the provider will not perform any invasive procedures) - REQUIRED
- ACLS - REQUIRED
- AHF Fellowship - LVAD experience - REQUIRED
- Provider Must be local - only mileage is reimbursed - REQUIRED
- DEA - REQUIRED
- Active VA license - REQUIRED
- Temps can be offered - must have clean background to be considered.- REQUIRED
