Position Overview Community Medical Services (CMS) is urgently hiring a Case Manager to help individuals suffering from addictions to opioids and other illicit substances gain access to care. Under the direct supervision of the Clinic Manager, you’ll serve as the primary contact for behavioral health services required outside of counseling, including new client intake, wellness check phone calls, clinical documentation, and insurance needs.

Schedule & Pay Schedule: Monday through Friday, 9:30 a.m. – 6 p.m.

Pay Range: $22–$26 per hour (depending on education)

Location: Lakewood on Colfax

Responsibilities

Coordinate internal support needs to reduce treatment barriers.

Screen potential clients, schedule intake appointments, and facilitate new client treatment program orientation.

Assist clients upon intake with appropriate documentation, including releases of information, coordination of care letters, client fee agreements, advanced directives, and language assistance forms.

Monitor Medicaid or other insurance paperwork for all clients, completing required intake and discharge forms as required.

Complete wellness checks via phone, maintaining progress notes and other required documentation for services provided as directed by the Clinic Manager and Clinical team members.

Coordinate care with PCP and other mutual providers (MH tx, DCS, probation/parole, etc).

Assist with transportation arrangements as needed.

Assist with completion of required intake documents and forms, collecting signatures and routing as appropriate.

Facilitate internal transfers to other CMS locations as well as transfers to external MAT programs.

Arrange for courtesy dosing and request exceptions for take‑home medication policy.

Collaborate with assigned counselors on follow‑up regarding AWOLs and abnormal drug screens.

Facilitate the application and tracking process for the Substance Use Disorder Voucher process and communicate with clients regarding the process.

Desired Experience

Experience working with multi‑cultural and low socio‑economic individuals.

Excited to work in an outpatient setting, similar to a doctor’s office.

Residential experience and knowledge of psychotropic medications.

Requirements

High school diploma or GED required.

At least one (1) year of combined behavioral health education and work experience.

Bachelor’s Degree in a related field preferred.

Valid state‑issued driver's license, 39‑month MVR clearance required.

Knowledge of behavioral management, de‑escalation methods, crisis intervention, and defusing techniques to prevent violent behavior.

Academic & working knowledge of DSM‑V, community resources, clinical methods and procedures and application of the same.

Bilingual in English/Spanish preferred.
