Position Type: Full-Time M F Schedule
Setting: PACE Program (serving seniors 55+ with chronic needs)
Overview
This role supports a PACE program designed to help seniors maintain independence at home while receiving a full continuum of medical and supportive services. The Medical Director provides clinical oversight, participant care supervision, and leadership within an interdisciplinary team.
Key Responsibilities
• Conduct history & physical exams, reassessments, and develop treatment plans
• Evaluate PACE applicants and collaborate in Intake & Assessment decisions
• Discuss prognosis and care planning with participants and caregivers
• Collaborate closely with nursing staff and interdisciplinary team members
• Provide clinical guidance to PCPs and APPs
• Participate in team/family conferences and care planning
• Educate participants and families on chronic conditions and health maintenance
• Order/interpret diagnostic tests and document findings
• Participate in provider on-call rotation
• Contribute to Quality Assurance Performance Improvement (QAPI)
• Support development/maintenance of medical policies and protocols
Skills & Background Needed
• Strong physical assessment and chronic disease management skills
• Ability to manage priorities based on participant needs
• Interest in alternative care models serving frail older adults
Submission Requirements
• Active Massachusetts physician license Required
• Board Certified in Internal Medicine or Family Practice Required
• Board Certification in Geriatrics Preferred
• DEA Registration Required
• Previous supervisory experience Required
• Minimum 1 year experience with frail or elderly populations Required
