About the Role The Primary Care Physician (PCP) is a licensed, Board Certified or Board Eligible physician in Internal or Family Medicine. You will play a key role on the clinical operations team, providing direct patient care in an ambulatory health-care center setting, and occasionally in acute care, nursing homes, skilled nursing facilities (SNF) and home settings depending on assignment needs.
Essential Job Duties / Responsibilities
Functions independently as a primary care practitioner as part of a patient care team.
Independently assesses acute and non‑acute clinical problems.
Performs and documents physical assessments and patient histories, analyzes trends in patient conditions, develops, documents and implements a patient management plan, and provides appropriate patient/family counseling and education.
Plans patient care based on in‑depth knowledge of the specific patient population and protocols, anticipating physiological and psychological problems commonly encountered, including cultural background, level of understanding, personality and support systems. Serves as patient advocate.
Handles patient management tasks: writing admission, transfer and discharge orders; ordering and interpreting laboratory and diagnostic studies; ordering medication and treatments; referring patients for appropriate specialties; documenting progress notes and summaries.
Participates in patient‑care rounds, conferences, and communicates patient‑management strategies to the team.
Collaborates with multidisciplinary team members to ensure strategies meet patient‑care needs.
Recognizes situations requiring immediate attention and initiates life‑saving procedures when necessary.
Uses advanced communication skills to solve complex situations and improve processes and services to patients.
Collaborates to analyze and evaluate current health‑care delivery systems and to implement new practice patterns.
Participates in outside activities that enhance personal and professional growth and development.
Initiates arrangements and writes orders for discharges, completing appropriate paperwork.
Works collaboratively with physicians, nurses, PT, social workers, family and key caregivers to transition patients to a lower level of care as medically appropriate.
Advocates and educates to ensure patients have required services and education.
Introduces self to patient/family and explains primary‑care provider role.
Facilitates patient/family conferences to review treatment goals, optimize resources, and provide education.
Enhances collaborative relationships to allow patients/families to make informed decisions re: goals of care, palliative care and hospice.
Manages resource utilization and reimbursement for services.
Facilitates discharge to appropriate level of care and uses preferred providers when additional services are required.

Education and Experience Criteria
Current licensure in the state of practice is required.
Certification in Basic Cardiac Life Support is required.
Board certification in Internal Medicine or Family Medicine preferred; required to achieve Board certification within two years.
Current DEA number for schedule II‑V controlled substances.
Prefer minimum of one (1) year clinical experience in geriatric, adult or family practice setting.
Compensation $214,700 - $306,714 Salary
