Job Summary: Initiates and ensures the quality of pharmacy and drug distribution services in all patient care areas. Assumes direct responsibility for direct medication distribution, IV admixtures, safety and efficacy of prescribed drug therapy in accordance with laws and regulations governing pharmacists and the practice of pharmacy by the appropriate state and federal agencies.

Practice in accordance with state and federal laws, rules, and regulations, including but not limited to the Massachusetts Board of Registration in Pharmacy, Department of Public Health, the FDA, and the DEA. Maintains certification and credentials in accordance with the Board and adheres to institutional policies and procedures.
Review and verify medication orders with consideration of patient-specific parameters (diagnosis, weight/BSA, age, allergies, organ function, labs, medication duplication, interactions). Clarify unclear orders with the prescriber in a timely manner.
Document and communicate pertinent information with the healthcare team and pharmacy staff to ensure continuity of care.
Provide drug information to physicians, nurses, and other health care professionals.
Check and verify sterile and non-sterile medications.
Supervise the activities of students, technicians, and residents within the pharmacist area of practice.
Promote a culture of patient safety by submitting medication-related events and near misses.
Complete daily pharmacy surveillance tool monitoring, including follow-up and documentation, as well as other daily patient care initiatives and relevant reports.
Work collaboratively with all pharmacists, pharmacy technicians, and pharmacy personnel. Uphold a culture of respect and teamwork across the Pharmacy Department.
Participate in onboarding and training of new hires and precept pharmacy students and/or residents through core clinical rotations or learning experiences.
Participate in department initiatives including formulary stewardship, presenting new medication requests at Pharmacy & Therapeutics Committee, creating and maintaining policies and procedures, conducting assessments of workflows, and developing departmental tools and resources.
Support departmental workgroups to improve process and quality outcomes.
Support the development and revision of departmental clinical guidelines.
Other duties as assigned.
Qualifications Required Qualifications
PGY-1 Residency OR 1-3 years relevant hospital experience.

Pay Range $59.74 - $75.76
The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
