$115,000 – $125,000/year (DOE) Physical Therapy Assistant (PTA):

$65,000 – $77,000/year (per-point basis) Sign-On Bonus:

Up to $50,000 (PT), up to $10,000 (PTA) Licensure: PT: Minnesota license required; Wisconsin license required or willingness to obtain (Chisago City role) PTA: Valid Minnesota PTA license required

Position Summary Wheeler Staffing Partners is seeking compassionate, patient-focused

Home Health Physical Therapists (PTs)

and

Physical Therapy Assistants (PTAs)

to deliver high-quality care in patients’ homes across multiple territories in Minnesota. These roles offer an opportunity to make a meaningful impact on patients’ mobility, independence, and overall quality of life. PTs provide assessments, develop treatment plans, and deliver evidence-based care, while PTAs implement treatment plans under the supervision of licensed PTs.

Key Responsibilities – PT & PTA Provide patient-centered physical therapy services in home health settings to improve mobility, strength, balance, and functional independence. Conduct assessments and develop individualized, goal-oriented care plans (PT role). Implement therapy plans under the direction of a licensed PT (PTA role). Educate patients and families on exercises, recovery techniques, home safety, and mobility strategies. Collaborate with interdisciplinary healthcare teams to coordinate care and optimize outcomes. Maintain accurate, compliant clinical documentation for all patient visits and progress notes. Participate in quality improvement initiatives and adhere to professional standards of care. Travel safely to patients’ homes within the assigned territory. Respond to on-call needs as required by the position.

Required Qualifications Physical Therapist Master’s degree in Physical Therapy from a CAPTE-accredited program. Minimum 9 months of licensed PT experience; home health or community-based experience preferred. Active Physical Therapy license in Minnesota; Wisconsin license required or willingness to obtain (Chisago City). Current CPR certification (must be maintained). Valid driver’s license and reliable transportation. Physical Therapy Assistant Associate’s degree from an accredited PTA program. Valid Minnesota PTA license, registration, or permit. Current CPR certification. Licensed driver with reliable transportation. Preferred Attributes Prior home health or community-based care experience. Strong communication, interpersonal, and time management skills. Ability to work independently while collaborating effectively within a healthcare team.
