Field Operations Specialist
to support care management teams through in-home visits, service coordination, and administrative support. This role plays a key part in ensuring older adults and individuals with disabilities receive timely, high-quality services that help them remain safely in their homes. Key Responsibilities Travel to members’ homes to conduct in-home care management visits. Communicate effectively with members, caregivers, and both clinical and non-clinical care team staff. Document all interactions accurately using the electronic medical record system. Schedule care management visits promptly and efficiently. Submit travel reimbursement requests accurately and on time. Review reports and manage assigned visit queues to ensure timely outreach and scheduling. Qualifications High School Diploma or GED required; BA/BS preferred. 2–3 years of experience in managed care, home care, or a related healthcare setting. Valid NYS driver’s license and ability to travel for home visits. Must be able to read, write, and speak English;
bilingual in Spanish required . Proficiency in MS Office. Strong customer service skills and the ability to respond to inquiries from members, regulatory agencies, or community partners. Compensation & Benefits Hourly pay:
