Our Client has an immediate need for a Bachelor Level Case Manager in Boston, MA. SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY

Provide clients with clinical services, including case management, individual treatment or group work Complete all documentation including assessments, Relapse Prevention Plans, and Individual Treatment Plans Oversee the intake process Determine appropriate transition and discharge plans for all residents Develop individualized behavior management plans and support staff to understand the individual youth treatment concerns Facilitate case conferences and discharge planning for all residents Assist with family engagement, permanency planning and life-skills development for all residents. Provide on-call support CLIENT'S REQUIRED SKILLS AND EXPERIENCE

Bachelors (Master's preferred) in social work or human services field At least 3 years of clinical experience Prior familiarity with Massachusetts child welfare systems and / or DCF Must have valid driver's license and vehicle DTG ADVANTAGES
