Job Description The Admissions RN organizes and directs hospice care/home health and is experienced in nursing, with emphasis on community health education and experience. The professional nurse builds from community resources to plan and direct services to meet the needs of individuals and families in their homes and communities. The role reviews hospice/home health qualification criteria, provides an overview of services to patients and families, maintains accurate clinical notes regarding patient condition and care, records pain/symptom management changes or outcomes, communicates with physicians regarding patient needs and reports changes in condition, obtains and receives physicians’ orders as required, and performs other duties as assigned.
Primary Duties and Responsibilities
Organize and direct hospice care/home health with emphasis on community health education and experience.
Build from community resources to plan and direct services to meet the needs of individuals and families in their homes.
Review hospice/home health qualification criteria and provide an overview of services to patients and families.
Complete, maintain, and submit accurate clinical notes regarding patient condition and care provided.
Record pain/symptom management changes and outcomes as appropriate.
Communicate with physicians regarding patient needs, report changes in condition, and obtain/receive physicians’ orders as required.
Perform other duties as assigned.
Qualifications Education and Experience
One (1) year of experience as a professional nurse within the last three (3) years; OR a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license.
Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.
License Required Current licensure as a Registered Nurse in good standing in the State of California.
Certifications Required Current BLS certification.
