R23127The Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) provides comprehensive primary and preventive medical care to geriatric patients in community-based settings, including patient homes and nursing facilities. Care is delivered in collaboration with a CHMG physician, with a focus on optimizing health, independence, and quality of life for older adults. This role requires travel to patient residences and long-term care facilities.30-hour role**Education**Master’s degree in Advanced Practice Nursing or completion of Physician Assistant program.**Certification, Registration & Licensure*** NH APRN or Physician Assistant license* Current DEA license* National certification in specialty area preferred**Experience*** Minimum of 2–3 years of clinical experience in geriatrics or primary care with older adults* Experience in home visits or nursing facility care strongly preferred* New graduates will not be considered for this role**Responsibilities*** Obtain detailed patient histories, perform physical examinations, and develop appropriate differential diagnoses for geriatric patients* Order, interpret, and follow up on diagnostic tests* Diagnose and manage acute and chronic conditions within the scope of practice* Prescribe and manage medications according to clinical guidelines and practice standards* Provide individualized patient education focused on preventive care, chronic disease management, and overall wellness for older adults* Maintain thorough, timely documentation of patient encounters in the medical record* Collaborate closely with CHMG physicians, nursing staff, and other care team members to ensure continuity of care**Key Attributes*** Strong communication and interpersonal skills to interact effectively with older adults, families, and care teams* Flexibility and independence in providing care across multiple settings* Commitment to high-quality, patient-centered geriatric careConcord Hospital is an Equal Employment Opportunity employer.
