Part Time / Weekend Physical Therapist Case Manager: Home Health 21st Century Home Health Services (21HHS) is the largest home‑health agency in San Francisco and the fastest‑growing in the Bay Area. Responsibilities

Perform home evaluations and develop individualized care plans.

Collaborate with Physical Therapy Assistants (PTAs) to implement care plans.

Assist patients with mobility, movement, and regaining independence.

Communicate and coordinate with physicians and interdisciplinary team members.

Evaluate and report patient progress to MDs.

Compensation Base pay: $120,000 – $135,000 per year, plus an annual $10,000 bonus. Eligible for a $20,000 bonus if promoted to PTII. Pay is commensurate with experience.

Qualifications

Current unrestricted CA Physical Therapist license.

Current BLS/CPR certification.

Valid CA driver’s license.

Knowledge of local community resources.

Understanding of patient development and human behavior related to illness/injury.

Knowledge of state and federal home‑care regulatory guidelines (preferred).

Preferred experience: home‑health care, high‑acuity patients, interdisciplinary teamwork, case management fundamentals.

Self‑directed, empathetic, critical thinker with outstanding interpersonal skills.

Leadership, organized, and able to work independently.
