Job Summary Bloom providers serve as the primary care provider for patients and see patients based on medical necessity. In addition to comprehensive primary care and care management, Bloom offers add‑on services such as on‑demand care, acute condition diagnosis and immediate clinical interventions, wound care, hospice, real‑time blood draws, and more. Patients are treated longitudinally through a holistic primary care model that includes screening and management of social determinants of health, patient and caregiver education, medication reconciliation, fall risk assessments, home food assessment, and advanced care planning. Our integrated data stack tracks and monitors patient hospitalizations via the Colorado health information exchange, enabling providers to see patients post‑discharge within 72 hours to ensure stabilization and clear instructions.
Responsibilities
Help elderly patients to age in place at home
Deliver comprehensive primary care at the patient’s residence
Assess and address socioeconomic status, housing stability, and cultural influences to optimize outcomes
Demonstrate exceptional communication and relationship‑building skills with internal clinicians and external partners
Provide data‑driven, condition‑specific specialty visits when needed
Manage health outcomes for an enplaned patient population
Utilize population health data to drive superior patient outcomes
Attend ongoing educational opportunities
Commute to assigned private residences to provide care

Scheduled days: Monday – Friday (5 days)
10 patients per day
Flexible schedules with rotating call (2–3 × yr) and weekends off

Qualifications
Master’s degree required
Licensure in the state of Colorado

Compensation The salary for this position is competitive and commensurate with experience. The total compensation range in Colorado is typically $100,000 – $120,000 annually, with potential performance‑based bonuses and other benefits.
