The Certified Diabetes Educator (CDE) will be responsible for diabetes education and care of patients and families in inpatient and outpatient settings, including children, adolescents, and young adults. The role covers new onset and established diabetes with Type 1 and Type 2, insulin injections and pumps, and oral medications. The CDE provides patient education, ongoing support, develops education and treatment plans, evaluates learning needs, and follows up to ensure learning goals are met. The position supports research‑based projects, maintains American Diabetes Association certification, participates in school‑based staff in‑service days, and attends diabetes clinics at Children's National satellite clinics.
Responsibilities
Perform complete educational and skills assessments for patients with Type 1 and Type 2 diabetes, including those on oral medications, insulin injections, and insulin pumps.
Provide individualized and group diabetic self‑management education, including meal planning, carbohydrate counting, exercise planning, daily schedule adjustments, and response to high/low blood sugar readings.
Follow up with patients to evaluate achievement of learning goals and assist families with decision‑making regarding diabetes self‑care.
Assist families with sick day management guidelines and coordinate resources.
Support research‑based projects and maintain CDE certification for Children's National’s Diabetes Education Program.
Participate in diabetes clinics as needed at all satellite clinic locations.
Collaborate with physicians, advanced practice nurses, and the diabetes team to review results, change therapy, and develop tool/forms per national standards.
Maintain current knowledge of diabetes self‑management products and educational materials.
Provide phone triage/point of contact for patients, schools, and community organizations on a daily basis.
Participate in daily morning huddles, departmental meetings, and diabetes team meetings.
Facilitate patient flow at diabetes clinics and download blood glucose meters as needed.
Recognize abnormal diabetes lab results and take appropriate action in collaboration with the MD.
Interpret the intent of diabetes policies in a clear and non‑biased manner.
Develop and maintain working relationships with diabetes product representatives in the community.
Participate in ADA Program recognition and attend annual Advisory Committee meetings.
Advance professional practice through active participation in ADA, AADE, and other organizations.
Role model conflict management and demonstrate teamwork and communication skills.
Qualifications
Minimum Education : Bachelor’s Degree in Nursing, Nutrition Science, or related (Required).
Minimum Work Experience : One year of pediatric diabetes related experience; experience managing patients on insulin pumps and use of CGM systems (Required).
Licenses and Certifications : Registered Nurse in District of Columbia (Required) or Licensed Dietitian/Nutritionist (Required). Certified Diabetes Educator (CDE) Certification through the American Diabetes Association, 1 Year (Required).
Functional Accountabilities :
Participate and promote family/patient and staff education, present at hospital, community, and national educational forums.
Educate patients/families with newly diagnosed and established diabetes per the 10 Standards of practice for Diabetes Educators.
Educate patients/families in the 10 content areas specified by the AADE.
Assist patient/family to educate other caretakers, including school staff, to ensure a safe transition to outpatient setting.
Provide on‑call diabetes education on weekends and holidays.
Obtain continuing education in topics specified by the National Standards.
