As a team member of our Senior Community Care (SCC) product, we work with a team to provide care to patients at home in a nursing home, assisted living for senior housing. This life‑changing work adds a layer of support to improve access to care. We're connecting care to create a seamless health journey for patients across care settings.

We serve millions of Medicare and Medicaid patients, and as a CCM Nurse Practitioner/Physician Assistant per diem you will provide care to Optum members and be responsible for the delivery of medical care services on a periodic or intermittent basis.

Primary Responsibilities

Primary Care Delivery

Deliver cost‑effective, quality care to assigned members

Manage both medical and behavioral, chronic and acute conditions effectively, in collaboration with a physician or specialty provider

Perform comprehensive assessments and document findings concisely and comprehensively to meet CMS regulations

Ensure all diagnoses are ICD‑10, coded accurately, and documented appropriately

Address all quality elements and document them

Conduct initial, annual, and post‑hospitalization medication reviews and reconciliations

Facilitate agreement and implementation of the member’s plan of care by engaging facility staff, families, and physicians

Evaluate and revise the plan as needed for effectiveness, necessity, and efficiency

Use clinical guidelines and CCM protocols

Attend and complete mandatory educational and LearnSource training requirements

Travel between care sites

Care Coordination

Understand payer/plan benefits and CCM policies, then articulate them to providers, members, and stakeholders

Assess medical necessity and effectiveness of ancillary services to determine benefit events

Coordinate care transitions across care settings

Monitor member and family needs, adjusting plans as circumstances change

Review orders and interventions for appropriateness and response to treatment

Evaluate cost‑effectiveness while meeting member, family, and provider needs

Program Enhancement Expected Behaviors

Communicate effectively with internal and external parties

Promote the CCM program in assigned facilities through partnership with stakeholders

Generate original thinking and creativity to improve methods and approaches

Work independently with minimal supervision

Enter available hours into the web‑based application at least one month in advance

Demonstrate initiative toward individual, team, and organizational goals

Participate in CCM quality initiatives

Check Optum email intermittently for required trainings, communications, and scheduling

Required Qualifications

Certified Nurse Practitioner through a national board

For NPs: Graduate of an accredited MSN program and board certified via AANP, ANCC, or relevant specialty

For PAs: Certified Physician Assistant through NCCPA

Active and unrestricted state license

Current DEA licensure/prescriptive authority (or ability to obtain post‑hire)

Ability to establish a collaborative practice agreement, if applicable

Physical ability to lift 30‑pound bags, navigate stairs, and move through various dwelling conditions

Valid driver’s license and reliable transportation to travel to client sites

Preferred Qualifications

At least 1 year of hands‑on post‑grad experience in long‑term care

Understanding of geriatrics and chronic illness

Knowledge of advanced illness and end‑of‑life discussions

Proficient computer skills for electronic medical record documentation

Ability to develop and maintain positive customer relationships

Adaptability to change

Employees must remain in an Advanced Practice Clinician or Physician role within Home & Community for 36 months to receive the full student‑loan‑repayment benefit

Compensation for this specialty generally ranges from $104,500 to $156,000. Total cash compensation includes base pay and bonus, and may increase over time based on performance.
