Tailored Care Manager Supervisor – RAL
EPIPHANY FAMILY SERVICES
is seeking a Tailored Care Manager Supervisor to lead a team of care managers coordinating whole-person care for individuals with Intellectual Developmental Disabilities (I/DD), Traumatic Brain Injury (TBI), serious mental illness, or severe substance use disorders.
Key Responsibilities
Team Leadership: Supervise, train, and mentor a team of Care Managers to ensure high-quality care and meet service requirements.
Whole-Person Care: Oversee care plans addressing physical health, mental health, developmental disabilities, substance use, and other unmet needs.
NC Medicaid Compliance: Ensure services meet North Carolina Medicaid waiver requirements and state policies.
Integrated Care: Promote integrated care teams and foster collaboration between providers to achieve better health outcomes.
Case Management Oversight: Review cases, ensure appropriate member assignments, and monitor care to close gaps.
Community Engagement: Work with community stakeholders, assess service needs, and identify gaps or opportunities to enhance services.
Quality Assurance: Conduct case record reviews to maintain high-quality services and develop new policies and procedures.
Education Master’s degree in a related field such as social work, psychology, nursing, or human services.
Licensure A clinical license (LCSW, LCMHC, LCAS) is required. Dependently licensed candidates are encouraged to apply.
Experience Two or more years of experience in care management, case management, or care coordination, especially with I/DD, TBI, or severe mental illness.
Skills Excellent clinical, interpersonal, organizational, and communication skills. Experience with electronic medical records and knowledge of DSM-5 are beneficial.
