We are looking for a dynamic Hospice RN to join our passionate team of Clinical Professionals!
We currently are looking to fill Full-time, Part-time and PRN in our Baltimore, Kensington and Lanham (Maryland); DC and Virginia branches.
The Clinical Nurse RN is responsible for ensuring quality total patient care through case management, assessment, interventions, and evaluation of patient needs through nursing care plans for each assigned patient.
Responsibilities
Performs initial and ongoing comprehensive assessments of the client’s needs at appropriate time points for home health care or hospice patients
Acts as a case manager, coordinating all patient/family services and prioritizing the patient needs with the members of the interdisciplinary team
Applies specific criteria for admission and re-certification to care, as applicable, to establish appropriate levels of care and patient’s eligibility
Initiates appropriate preventive and rehabilitative nursing procedures, and provides referrals to other services and disciplines as needed, specialized nursing skill (such as IV), and effective symptom management
Accurately and completely prepares all clinical documentation per agency policy
Identifies and communicates changes in the patient’s condition; informs the physician and obtains new orders as needed
Maintains communication with the patient, family and other members of the home care team to ensure coordination of services and episode management that meets patient goals in a cost-effective manner
Evaluates outcomes of care through performance improvement activities, clinical record review, and other assigned duties
Supervises LPN/s and paraprofessionals providing home health care or hospice services
Participates in care conferences, staff meetings and at least quarterly, in-service programs
Plans for the discharge from service or bereavement follow-up after hospice death
Qualifications
Must have a Compact RN License and/or Registered Nurse licensed in the state(s) of operation, required
Must possess sound knowledge of nursing principles, procedures, and standards of practice as determined by state and federal regulations, required
A minimum of one year of licensed professional nursing experience is preferred;
Experience in community or home health preferred
