Job Summary The Aging Case Management Program offers case management and long‑term housing subsidy services that assist individuals experiencing age‑related decline in health or needing help with resources navigation and service connections to reduce risk of institutionalization. The long‑term rent assistance program provides monetary help and case management to eligible clients to prevent eviction and/or homelessness, empowering seniors and the impaired adult population to increase independence and age in place.
Job Title Program Manager, Housing
Reports to Associate Deputy Director
Division Aging Support Services
Position Summary Under general supervision from the Associate Deputy, the Program Manager, Housing assumes responsibility for day‑to‑day management of long‑term housing subsidy services, works with case management staff, provides leadership and direct programming oversight, and carries a caseload of at least 20 cases focused on core housing case management, benefits advocacy, and support services for physical and mental health.
Core Responsibilities
Develop, maintain, and improve standards for client services and written procedures and protocols.
Monitor and direct day‑to‑day operations for the assigned service area in compliance with Catholic Charities policies, state licensing requirements, and contract funder specifications.
Assign and manage a caseload portfolio of up to 20 clients.
Build and maintain positive professional relationships with collaborative service providers, funders, and community partners.
Supervise, train, and provide support to 2–3 staff; oversee client discussions, assessments, care planning, case closure, and aftercare.
Participate as a member of the organization‑wide Management Team to deliver high‑quality services aligned with the mission, vision, and values of Catholic Charities.
Enter data into appropriate database systems and complete monthly case management reports.
Develop and implement strategic plans, set objectives, gather input, and adjust plans as needed.
Assist in acquisition and renewal of foundation grants and government contracts; support proposal and reporting activities.
Participate in meetings, training programs, client peer reviews, and other required professional development activities.
Provide practical support, education, and assistance to tenants to address immediate needs related to stability and life‑management skills, with a minimum of monthly client contact.
Coordinate with Catholic Charities CYO programs and community agencies to ensure quality service delivery.
Offer information and referral for services, client contact, and assistance in meeting service plan goals.
Comply with program policies and procedures to protect client and caregiver interests and facilitate smooth program operation.
Maintain accurate, current, and complete client files and complete all required data collection forms and reports.
Coordinate with collaborative agencies to identify and intervene in client issues that may destabilize housing.
Interface with other agencies and departments to ensure client compliance with program requirements.
Make recommendations to enhance and improve services.
Perform other duties as assigned and as needed.
Education & Experience
Bachelor’s degree in Social Work or a related field with a minimum of three (3) years of directly related case‑management experience.
Substantial or demonstrated knowledge/experience working with families in crisis or unstable housing situations.
Minimum three years of experience providing client case‑management services with the aging population and/or service programs.
Experience in delivering health, social, and/or aging services in the community, especially with diverse communities and knowledge of community resources.
Leadership abilities, teaching experience, and the ability to take charge or make decisions as needed.
Certified in Red‑Cross basic first aid and CPR required, obtainable within 30 days of hire.
Knowledge, Skills & Abilities
Excellent interpersonal and communication skills.
Knowledge of San Francisco County social services networks.
Sensitivity and knowledge of services for seniors and adults with disabilities, homelessness/unstable housing, substance use disorders, and associated healthcare issues.
Certified or familiar with HCV/Housing Quality Standards.
Experience interacting with individuals from diverse backgrounds to stabilize or improve housing situations.
Preferred bilingual proficiency in Mandarin/Cantonese or Spanish.
Functional knowledge of Microsoft Office products.
Ability to perform duties with minimum supervision.
Clear, concise verbal and written communication skills.
Professionalism and ability to work under pressure and as part of a team.
Valid CA driver’s license and personal car preferred.
Additional Qualifications
Knowledge of the structure and content of services to seniors.
Strong problem‑solving skills and ability to follow through.
Strong telephone skills.
Computer literacy.
Respect and sensitivity for cultural differences; promotion of a harassment‑free environment.
Professionalism—approach others tactfully, react well under pressure, treat others with respect, and accept responsibility for own actions.
Attendance at required meetings, supervision, and other professional development activities.

TB screening – negative test required
Access to accurate and timely data entry for performance and quality improvement reporting.
Submission of case records in a timely manner.
Valid California driver’s license required if driving is required.
Physical Demands
Frequent lifting, pushing, pulling, bending, standing, stooping, kneeling, reaching, twisting, and walking.
Medium work: up to 50 lbs occasionally, up to 20 lbs frequently, up to 10 lbs constantly.
Close visual acuity required for analyzing data.
No driving required; if required, provide valid driver’s license and proof of insurance.
Exposed to prayer, scents, animals, latex gloves, bleach solution, and outside weather while monitoring clients.
Environmental Conditions
Moderate office noise level.
Potential exposure to perfume, latex gloves, bleach solution, and animals (dogs and cats).
Occasional exposure to outside weather.
