Residential Habilitation Specialist/QIDP

This position will serve as QIDP for assigned sites. Being fully responsible for the development of Residential Staff Action Plans and Individual Plans of Protections (IPOPs) in accordance with the Life Plan.; participating in Life Plan meetings and maintaining open lines of communication with IRA staff, Care Coordinator, and treatment team on the person’s desired valued outcomes. Will also assist the Site Manager with training DSPs on Staff Action Plans and monitoring the implementation of Staff Action Plans and the individual’s response to service.
Attends Life Planning Meetings for assigned individual. Participates as an active member of the program planning team .
Develops and writes all Staff Action Plans for assigned sites within regulatory timeframes. Ensures all plans are reflective of the goals/valued outcomes listed in the Life Plan.
Provides oversight of the Staff Action Plan implementation along with daily documentation, ensuring the plan continues to meet the individuals’ needs..
Attends IRA staff, action planning, and medical meetings that pertain to the implementation of the Staff Action Plans and Individual Plans of Protection (IPOP).
Conducts training of staff at assigned IRA’s in regard to Staff Action Plans, service documentation, and IPOP.
Ensures the ongoing safety of all staff and individuals by maintaining a safe environment as well as reporting any concerns to the Program Coordinator and/or other appropriate staff member.
Education and Experience Requirements
Meets regulatory requirements for QIDP status which, aside from a few listed exceptions, requires at least a Bachelor’s Degree in approved an approved degree along with one (1) year experience of OPWDD experience (QIDP status) Skills, Knowledge and Abilities
Knowledge of developmental disabilities. Good oral and written communication skills and basic computer skills. The ability to read and write case notes as well as understand written directions. The ability to self-initiate, organize, schedule and analyze tasks. Certifications – Must be or become:
CPR/First Aid certified. Full SCIP-R certified. Verifications and Screenings
OPWDD clearance for convictions of criminal offenses. County criminal background check. Clearance through NYS Central Child Abuse and Maltreatment Register. A drug screen prior to the commencement of employment. Tuberculosis screening started or completed prior to the commencement of employment. Approval of NYS Justice Center. Be authorized as an Agency Driver by
Holding and maintaining a valid NYS Driver’s License. Maintaining an approved driving record according to Agency Policy and Procedure. Use of personal vehicle for work purposes; travel between work locations is required. Physical and Mental Requirements
Seeing, hearing, walking, sitting, standing, driving, stooping, bending, carrying, stretching, and climbing stairs as well as the ability to perform all required movements required by the SCIP-R curriculum. Lifting up to fifty (50) pounds. Exercising independent judgment, concentrating, thinking, communicating, interacting with others, managing multiple tasks and coping with stressful situations and deadlines. Staying awake and alert while on duty.
