Responsibilities
Perform all work in a manner consistent with NYAP’s mission, values, and philosophies.
Provide children with a positive and appropriate medical environment consistent with NYAP’s mission, values, and philosophies.
Ensure confidentiality of health records and adhere to NYAP, State, and Funder guidelines for protection of protected health information (PHI).
Work to obtain Memorandums of Understanding (MOUs) with medical, dental, and mental health providers.
Participate in intakes and discharges of children, including administering COVID‑19 testing, preparing medical documentation packages, and uploading documents in portals and health record systems.
Continuously assess children’s medical, dental, and mental health needs and coordinate care in accordance with NYAP policies and procedures.
Collaborate with children’s assigned cross‑functional team and providers to ensure appropriate care, including scheduling appointments, preparing treatment authorization requests, coordinating transportation, and obtaining medical clearances.
Report medical emergencies via the SIR/CLE process in accordance with NYAP and other policies.
Elevate medical concerns to the supervisor, cross‑functional team, providers, and the Division of Health for Unaccompanied Children (DHUC) for consultation.
Support foster parents by providing education on medical policies, answering questions, and assisting with required medical responsibilities.
Collect and upload health records from providers, ensuring completeness and compliance with confidentiality requirements.
Maintain medicine cabinets and first aid kits with essential supplies.
Provide in‑service training and ongoing education to staff and foster parents on health‑related needs.
Maintain Red Cross CPR/First Aid Trainer certification.
Assist and support program leadership during audit processes.
Perform other duties as requested.
Minimum Qualifications
Licensed Practical or Vocational Nurse (LPN/LVN) or higher health‑related qualification, or Bachelor’s degree in a relevant field.
Bilingual fluency in English and Spanish required.
Minimum of one year experience in public health or healthcare with demonstrated care coordination, health administration, or direct clinical care.
One year of experience in the child welfare field working with children and/or adolescents in a social service setting.
Valid driver’s license, clean driving record, safe personal vehicle, and active automobile insurance.
Ability to maintain compliance with agency‑required training programs.
Proficient use of desktop and laptop computers, smartphones, tablets, printers, fax machines, photocopiers, and software including word processing, spreadsheets, and databases.
Other Skills
Experience scheduling medical appointments.
Experience documenting medical appointments.
Recognizes and understands the needs of the immigrant population, providing support and creating a safe environment for children in the program.
Demonstrated ability to maintain liaisons with community health providers and expand community health network.
Willingness to work non‑traditional hours and be available for on‑call phone consultations as needed.
Physical Demands
Use of manual dexterity, tactile, visual, and audio acuity.
Repetitive motion, prolonged periods of sitting and standing, sustained visual and mental applications.
Occasional lifting (up to 25 pounds), bending, pulling, and carrying.
Ability to travel frequently and drive for extended periods, frequency varying by program demands.
