NY State LPN License (required) Minimum one year of Med/Surg experience in a hospital or long-term care facility NY State Driver’s License and reliable transportation Good computer skills for electronic documentation Compassionate, patient-focused attitude and strong communication skills What You’ll Do
Visit patients in their homes to implement and evaluate care plans Provide skilled nursing care and emotional support to patients recovering from illness, injury, or surgery Document visits electronically and complete all required documentation within 24 - 48 hours Collaborate with the Coordinator of Care and other team members to ensure high-quality, personalized care Contribute to a positive, patient-centered care experience and support overall health outcomes
