Overview Landmark Medical Center has earned “A” ratings for patient safety from The Leapfrog Group and numerous Healthgrades awards for patient safety excellence, heart care, and orthopedics. Established in 1873, the hospital serves northern Rhode Island and southern Massachusetts.

Work Hours 32 hrs/week, 8:00 am – 4:30 pm (Day shift)

Responsibilities

Provide emotional support, crisis intervention, and counseling for patients across all age groups and their significant others.

Conduct psychosocial assessments and assist in development and re‑evaluation of treatment plans as part of a multidisciplinary team.

Provide case management, milieu management, information referral, and coordination of discharge planning.

Maintain clinical records and documentation for each client during their length of stay.

Manage placement requirements and discharge planning responsibilities.

Qualifications – Required

Master’s Degree in Social Work or related field, or Master’s in Marriage and Family Therapy or related field.

State registration or license within 3 months of hire (or as required at the time of hire). Must maintain licensure.

Minimum one year of hospital or outpatient experience with emphasis in mental health care.

Experience working with a diverse population facing issues such as death and dying, maternal and child care, substance abuse, disability, chronic illness, etc.

Current state license (LSW or higher) required upon hire and must be maintained.

Qualifications – Preferred

Current BCLS (AHA) certificate upon hire.

Employment Status & Shift Part‑time (59 hrs+). Days.
