Overview Paradise Valley Hospital is the South Bay’s oldest hospital, the second oldest hospital in all of San Diego County, and the largest employer in National City. For more than 100 years, it has served San Diego faithfully, providing a wide range of inpatient and outpatient services, including an acute care hospital, the San Diego Spine & Joint Center, the Paradise Health & Senior Center, a cardiac catheterization lab, behavioral health services, and a 24‑hour emergency department.
Responsibilities Responsible for the coordination of various activities within the Case Management Department under the direction of the assigned Case Manager/Social Worker. Assist with development and implementation of discharge plans, case management referrals and authorizations, Medicaid state submission forms, and discharge planning by interviewing patients/families and gathering data to develop safe patient‑focused discharge plans.
Qualifications Required Qualifications
Knowledge of discharge planning, utilization management, and case management terminology and functions, in both managed care and non‑managed care environments.
Basic to intermediate computer skills.
High School Diploma.
Behavioral Violence Prevention Training within 3 months of hire and maintenance.
Preferred Qualifications
Current BCLS (AHA) certificate (required upon hire).
Associate’s degree or LVN license.
Experience in a hospital setting.
Pay Transparency Competitive compensation: $24.50 to $38.60 per hour, based on experience and qualifications.
