Position: Clinical Supervisor – Nursing Coverage area: Marin County
This leadership role reports to the Director of Nursing and requires field visits to patients’ homes. The position is a hybrid of field work and time spent at the 21HHS office.
Office:
2 days per week for administrative and team‑management duties.
Fieldwork:
3 days per week (average 12+ daily visits) conducting supervisory visits and providing direct support to RN Case Managers.
Responsibilities
Oversee clinicians in the field who provide care and treatment to patients in their homes.
Handle incoming calls promptly and ensure appropriate actions meet internal and external customer needs.
Coordinate training and educational programs for nurses to improve clinical outcomes and competencies.
Assess staff learning needs, formulate specific plans and goals for the next evaluation period, and serve as a role model and resource.
Act as the first point of contact for field clinicians to assist with patient‑care escalations.
Coordinate bi‑weekly Case Conferences to discuss patient progress and revise care plans.
Review and evaluate each case by examining services provided, holding conferences, and conducting record reviews; provide guidance to clinicians for improved performance.
Review patients’ clinical diagnosis, medications, procedures, and clinical course.
Assist clinicians in establishing immediate and long‑term therapeutic goals, setting priorities, and developing care plans.

Qualifications
Two years of management or supervisory experience within the past five years in a health‑care setting, preferably in Home Health.
Bachelor’s degree in Nursing (preferred) from an accredited school; graduate of an accredited nursing program.
Current California Registered Nurse license (RN).
Current Basic Life Support (BLS) card.
Valid California driver’s license with an acceptable driving record.
All licenses and certifications must be current at time of hire and sustained throughout employment.
