Active and unrestricted LPN license in the state of New York
Recent graduates are encouraged to apply
Skilled in medication administration and patient care procedures
Strong documentation, time management, and communication skills
Comfortable using EHR and digital charting systems
What you’ll do
Deliver direct nursing care to residents in skilled nursing and long‑term care settings
Administer medications and IVs as prescribed
Implement and evaluate individualized care plans
Record vital signs and monitor patient conditions
Supervise and guide other nursing personnel when requiredEducate patients, families, and caregivers as directed
